ACT Form CES-D collects ACT participant responses to items for the Center for Epidemiology Studies Depression (CES-D) Scale. These items are collected at study baseline (intake visit) when administering Form 73 and at follow-up visits through administration of a Form 23. An overview of the items is shown below.

Important Note: The 11-item CES-D has been collected over the duration of ACT (1994-present), but the 20-item CES-D has only been collected since 2001.
