ACT Form 68 Summary
Data Description
If an ACT participant was unable to undergo one of their biennial visits, then biennial visit data forms were not collected. Instead, a check-in telephone visit was offered to collect a limited set of information. Form 68 – Telephone Follow-up was one of the forms used for this data collection.
