ACT Form 74 Summary

Data Description

ACT Form 74 collects comments from ACT participants regarding their opinions of the study visit administration. A high-level overview of the content areas is shown below.

Feedback

• Was the exam too long?
• Were there any parts of the exam that you felt were difficult to perform?
• Were there any questions that you would rather not answer because they were too personal or sensitive?
• Were there any questions that were confusing or difficult to understand?
• Did you feel comfortable in the presence of our staff?
• Was it easy to understand the interviewer?
• While in the clinic, was the room temperature comfortable?
• Did you have any problems finding our clinic or parking?
• Do you have any other comments or suggestions about the examination?

 

Important Note: Form contents have changed over the years so all the above items may not be collected over the full duration of the ACT study.

This Form 74 dataset contains one row per participant's baseline visit at which the study form was collected.
