ACT Form 75 Summary

Data Description

ACT Form 75 collects self-reported demographic information on ACT participants. A high-level overview of the content areas is shown below.

General Demographics

  • Education and degree information
  • Gender
  • Race, Ethnicity
  • Marital status information
  • Living situation (who live with, how many people) 

Childhood History

  • Living situation (how many other children, how many people in house, how many bedrooms, location)
  • Family’s financial situation, were basic needs met, could family avoid small luxuries
  • Parental smoking status

Occupation / Income

  • Employment, working for pay
  • Retirement status (including age and reasons)
  • Volunteer work
  • Yearly income, financial situation (basic needs, luxuries)

Parents' Education

  • Years of school and highest degree earned by father
  • Years of school and highest degree earned by mother

Important Note: Form contents have changed over the years so all the above items may not be collected over the full duration of the ACT study. 


Data Structure 

This Form 75 dataset contains one row per visit at which the study form was collected.
