ACT Form 82 Summary

Data Description

ACT Form 82 collects study interviewer assessment information of the ACT participants at study baseline (intake visit). A high-level overview of the content areas is shown below.

Assessment of Participant

  • Impairment of hearing, vision, speech that interfered with the examination
  • Participant was awake and alert, seemed oriented
  • Participant’s affect and behavior appropriate and normal 
  • Participant appeared to show signs of Parkinsonism
  • Exam completeness

 

Important Note: Form contents have changed over the years so all the above items may not be collected over the full duration of the ACT study. 


Data Structure 

This Form 82 dataset contains one row per participant’s baseline visit at which the study form was collected.
