ACT Form CES-D Summary

Data Description

ACT Form CES-D collects ACT participant responses to items for the Center for Epidemiology Studies Depression (CES-D) Scale. These items are collected at study baseline (intake visit) when administering Form 73 and at follow-up visits through administration of a Form 23. An overview of the items is shown below.

11-item CES-D

Would you say in the last week… choose one of the responses (rarely or none of the time (<1 day), some or a little of the time (1-2 days), occasionally or a moderate amount of the time (3-4 days), most of the time (almost every day), don’t know / refused):

  • You were bothered by things that usually don’t bother you
  • You did not feel like eating, your appetite was poor
  • You had trouble keeping your mind on what you were doing
  • You felt depressed
  • You felt that everything you did was an effort
  • You were hopeful about the future
  • You felt fearful
  • Your sleep was restless
  • You were happy
  • You felt lonely
  • You could not get going

20-item CES-D

The 20-item form includes all 11 items above and the following 9 additional items. 

Would you say in the last week… choose one of the responses (rarely or none of the time (<1 day), some or a little of the time (1-2 days), occasionally or a moderate amount of the time (3-4 days), most of the time (almost every day), don’t know / refused):

  • You felt you could not shake off the blues, even with help from your family or friends
  • You felt that you were just as good as other people
  • You thought your life had been a failure
  • You talked less than usual
  • People were unfriendly
  • You enjoyed life
  • You had crying spells
  • You felt sad
  • You felt that people disliked you

 

Important Note: The 11-item CES-D has been collected over the duration of ACT (1994-present), but the 20-item CES-D has only been collected since 2001.


Data Structure 

This Form CES-D dataset contains one row per visit at which the study form was collected.
